Harvest for All: Applications for the Harvest for All program are live and due by 11:59 p.m. on May 1, 2020. Complete this form with donations collected from May 1, 2019 through April 30, 2020.

 Following the final tabulation of donations, the top chapter in each section will be recognized and statewide winners will receive the following prizes:

1^st place - $150

2^nd place - $125

3^rd place - $100

4^th place - $75

5^th place - $50
